Patients are typically referred to nephrologist by their primary care physician or other treating physicians. Our office staff will work closely with your doctor's office to ensure that we have the most accurate information on your current medical condition, including the results of any recent medical tests. If required by your insurance carrier, we will request that your primary care physician secure the appropriate prior authorizations from your insurance company.

Our office will contact you upon receiving the information from your doctor's office. A new patient packet  can be printed by you from this website to provide you the opportunity to complete the required forms in advance and bring or mail in the packet before your visit.  Please click on the two links below (new patient packet and new patient medical history) to get the necessary forms for your appointment.


Please bring the following information to your first appointment with our nephrologist:
 

All of your prescription bottles, including any over-the-counter medications
All of your current insurance cards and valid ID
All completed forms from the new patient packet
